If you have moved home, please visit the register to vote pages on the GOV.UK website and register again.

Change of name

Please provide details about your change of name below. You will need to upload photo evidence of this change.

Upload evidence

We need to receive evidence of your change of name. This can be a scanned copy or photograph of your marriage or civil partnership certificate, overseas marriage or civil partnership certificate, amended birth certificate or deed poll.

The image you upload must clearly show the change, be in JPG, PNG or PDF format and be less than 9MB in size.





Aged 76 or over

The HM Courts and Tribunals Service use the electoral register to identify people eligible (75 years or under) for jury service. If you are aged 76 or over and would like our records to be updated so that you are no longer contacted, please fill in your details below.
