Before you start

Before we disclose your personal data, we will need you to prove your identity. This can be a recent utility bill, a driving licence or passport. You will need to upload this evidence towards the end of this form. Please have this evidence ready before completing this application.
If you are acting on behalf of the applicant, you will also need to upload evidence of your authority to act on their behalf. This can be a letter from the person or evidence of a power of attorney.
